Fresh reports claim Iran’s Supreme Leader, Mojtaba Khamenei, suffered severe injuries in the February 28 US-Israel strike, raising major questions over Iran’s power structure and decision-making

Tehran: Iran’s newly appointed Supreme Leader, Mojtaba Khamenei, is reportedly alive but seriously injured, according to multiple international media reports citing unnamed Iranian officials. Claims suggest he suffered grave wounds during the February 28 U.S.-Israeli strike that killed his father and predecessor, Ali Khamenei.

The reports allege Mojtaba Khamenei has remained out of public view since assuming power, communicating only through handwritten messages while recovering under heavy security. If accurate, the developments could signal one of the most unstable leadership transitions in modern Iranian history.

Severe Burns, Speech Problems and Surgeries Reported
According to accounts summarising a report by The New York Times, Khamenei suffered severe burns to his face and lips, making speech difficult. The same reports claim he underwent surgeries on one leg and one hand, and may require a prosthetic leg as well as plastic surgery later.
“Gravely wounded, but mentally sharp,” one summary of the report stated.
No independent public medical confirmation has been issued by Iranian state authorities.

Why He Has Not Appeared in Public
Since succeeding his father, Mojtaba Khamenei has reportedly not delivered any verified live speeches or public appearances. That absence has fueled intense speculation about both his health and his ability to exercise full authority over Iran’s government and military institutions.
Reports also state that access to him has been sharply restricted over security fears, including concerns that enemies could track his location.
Military Figures Taking Key Decisions
International coverage claims that major wartime and strategic decisions are increasingly being handled by commanders linked to the Islamic Revolutionary Guard Corps rather than by Khamenei directly. If true, that would represent a major shift in how Iran’s power system traditionally functions.

Meanwhile, Masoud Pezeshkian and senior ministers are reportedly taking a larger administrative role during the uncertainty.

Leadership Rift Inside Iran
Analysts say prolonged silence from the Supreme Leader can deepen factional divisions within Iran, especially on questions involving negotiations with the United States, military responses, and internal governance.

Whether Mojtaba Khamenei eventually reappears publicly after treatment or whether military hardliners continue gaining influence behind the scenes remains one of the biggest geopolitical questions in the region.
